Le Bureau du budget du Congr\u00e8s estime que 700&nbsp;000&nbsp;personnes perdront leur couverture d&rsquo;ici 2034, dont 70&nbsp;% pour des raisons administratives (par exemple, des formalit\u00e9s non remplies, des probl\u00e8mes de communication, des difficult\u00e9s \u00e0 s&rsquo;y retrouver dans le syst\u00e8me), et non parce qu&rsquo;elles ne remplissent pas les conditions requises.<em><sup><a href=\"https:\/\/ccf.georgetown.edu\/2025\/08\/14\/new-cbo-health-coverage-estimates-of-budget-reconciliation-law\/\" type=\"link\" id=\"https:\/\/ccf.georgetown.edu\/2025\/08\/14\/new-cbo-health-coverage-estimates-of-budget-reconciliation-law\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">13<\/a><\/sup><\/em> Chaque formulaire non renvoy\u00e9, chaque appel rest\u00e9 sans r\u00e9ponse ou chaque barri\u00e8re linguistique repr\u00e9sente une perte de r\u00e9mun\u00e9ration forfaitaire pour un adh\u00e9rent qui \u00e9tait pourtant encore admissible.<\/p>\n\n\n\n<p><strong>Les exigences des \u00c9tats fragmentent le paysage de la conformit\u00e9.<\/strong> En 2025, 37&nbsp;\u00c9tats ont pr\u00e9sent\u00e9 de nouvelles lois sur l\u2019\u00e9quit\u00e9 en sant\u00e9.<em><sup><a href=\"https:\/\/www.healthscape.com\/insights\/states-are-staying-course-health-equity-policy-three-ways-health-plans-can-align\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">11<\/a><\/sup><\/em> Le programme californien CalAIM, qui impose l&rsquo;accr\u00e9ditation en mati\u00e8re d&rsquo;\u00e9quit\u00e9 en sant\u00e9, une prise en charge globale de la personne et une communication multilingue, incarne la forme la plus aboutie d&rsquo;une tendance qui se g\u00e9n\u00e9ralise chez les op\u00e9rateurs pr\u00e9sents dans plusieurs \u00c9tats.<\/p>\n\n\n\n<p>Il ne s&rsquo;agit pas l\u00e0 de d\u00e9fis s\u00e9par\u00e9s. Ce sont les m\u00eames ph\u00e9nom\u00e8nes. Ces trois changements de politique indiquent que la fa\u00e7on dont les r\u00e9gimes doivent g\u00e9rer les actions des membres est en train de changer en profondeur. Ce que les membres font r\u00e9ellement, ou ne font pas, se refl\u00e9tera directement dans les comptes de r\u00e9sultat. Le comportement des membres est le prochain grand levier de r\u00e9duction des co\u00fbts, mais c\u2019est aussi le seul que les approches traditionnelles n\u2019ont pas r\u00e9ussi \u00e0 influencer \u00e0 grande \u00e9chelle.<\/p>\n\n\n<div class=\"separator--long   \">\n    <hr>\n<\/div>\n\n\n\n<p><\/p>\n\n\n\n<h2 class=\"wp-block-heading has-praise-shadow-color has-text-color has-title-four-font-size\"><strong>Le co\u00fbt de l&rsquo;inaction est mesurable<\/strong><\/h2>\n\n\n\n<p>Les enjeux sont consid\u00e9rables si les responsables ne prennent pas de mesures d\u00e8s maintenant. Les d\u00e9penses m\u00e9dicales \u00e9vitables s&rsquo;\u00e9l\u00e8vent \u00e0 environ 79&nbsp;milliards de dollars par an, principalement en raison d&rsquo;assur\u00e9s qui ne comprennent pas leurs prestations, ne consultent pas leur m\u00e9decin traitant et ne prennent pas en charge leurs maladies chroniques. 40&nbsp;% des b\u00e9n\u00e9ficiaires de Medicare restent syst\u00e9matiquement en dehors du circuit des soins pr\u00e9ventifs de base.<em><sup><a href=\"https:\/\/paretointel.com\/insight\/medicare-advantage-rewards-programs-member-engagement\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">6<\/a><\/sup><\/em><\/p>\n\n\n\n<p>Et les approches traditionnelles visant \u00e0 r\u00e9soudre ces probl\u00e8mes s&rsquo;av\u00e8rent insuffisantes. Les portails traditionnels fournissent des informations, mais pas de conseils, car ils ne savent pas ce qu\u2019un membre doit faire ensuite, et ils ne peuvent pas communiquer avec lui dans la langue ou par le canal qui l\u2019inciterait r\u00e9ellement \u00e0 passer \u00e0 l\u2019action. La communication sur support papier passe \u00e0 c\u00f4t\u00e9 des populations que ces r\u00e9gimes visent principalement&nbsp;: des adh\u00e9rents multilingues, pour qui l&rsquo;utilisation d&rsquo;un appareil mobile est prioritaire et qui manquent de temps, pour lesquels un formulaire envoy\u00e9 par la poste ne constitue pas un moyen de passer \u00e0 l&rsquo;action. Les centres d&rsquo;appels ne sont pas en mesure de s&rsquo;adapter au volume et \u00e0 la complexit\u00e9 des demandes de ces populations.  <\/p>\n\n\n\n<p>Ces co\u00fbts op\u00e9rationnels s\u2019accumulent et finissent par avoir un impact clinique. En l&rsquo;absence d&rsquo;un syst\u00e8me centralis\u00e9, les services se retrouvent \u00e0 effectuer plusieurs fois le m\u00eame travail&nbsp;: des communications redondantes sur diff\u00e9rents canaux et un volume r\u00e9p\u00e9t\u00e9 d&rsquo;appels de la part de membres qui n&rsquo;ont pas trouv\u00e9 de r\u00e9ponse d\u00e8s leur premier contact. \u00c0 la fin, les r\u00e9sultats en p\u00e2tissent.<\/p>\n\n\n\n<p>Les leviers traditionnels ne peuvent pas non plus combler l&rsquo;\u00e9cart. Les programmes de gestion de l\u2019utilisation des soins, d\u2019orientation dans le r\u00e9seau et de coordination des soins ont permis de stabiliser les tendances, mais ils ne peuvent plus produire le changement significatif n\u00e9cessaire pour pr\u00e9server les marges.<em><sup><a href=\"https:\/\/www.engagys.com\/insights\/engagement-is-the-new-cost-lever\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">7<\/a><\/sup><\/em> <\/p>\n\n\n\n<h3 class=\"wp-block-heading has-title-four-font-size\"><strong>L&rsquo;exp\u00e9rience des membres repose sur l&rsquo;infrastructure op\u00e9rationnelle<\/strong><\/h3>\n\n\n\n<p>Chacun des changements de politique mentionn\u00e9s ci-dessus se r\u00e9sume au m\u00eame probl\u00e8me&nbsp;: amener le bon intervenant \u00e0 prendre la bonne mesure au bon moment. Les \u00e9toiles \u00e9voluent lorsqu\u2019un membre effectue une visite de pr\u00e9vention ou comble une lacune dans son parcours de soins. Les recettes de Medicaid sont maintenues lorsqu&rsquo;un assur\u00e9 admissible renouvelle son adh\u00e9sion avant la date limite. Et aucun portail, centre d&rsquo;appels ou solution ponctuelle n&rsquo;a \u00e9t\u00e9 con\u00e7u pour y rem\u00e9dier \u00e0 grande \u00e9chelle.  <\/p>\n\n\n\n<p>Ce qui manque, c&rsquo;est une plateforme native d&rsquo;IA dot\u00e9e d&rsquo;une orchestration intelligente, capable de d\u00e9terminer la marche \u00e0 suivre et de mettre en relation la situation particuli\u00e8re d&rsquo;un adh\u00e9rent avec les mesures sp\u00e9cifiques qui permettent de r\u00e9duire les co\u00fbts, d&rsquo;\u00e9tendre sa couverture et d&rsquo;am\u00e9liorer son \u00e9tat de sant\u00e9. Cela permet de regrouper toutes les informations dont dispose le r\u00e9gime sur chaque adh\u00e9rent, notamment en mati\u00e8re d&rsquo;admissibilit\u00e9, de demandes de remboursement, de contextes cliniques et de d\u00e9terminants sociaux, afin de dresser un portrait global de chaque personne. Le changement de m\u00e9thodologie des \u00e9toiles est inscrit dans la r\u00e8gle d\u00e9finitive pour l&rsquo;ann\u00e9e civile&nbsp;2027. Les exigences en mati\u00e8re de r\u00e9\u00e9valuation pr\u00e9vues par la loi H.R.&nbsp;1 entreront en vigueur en d\u00e9cembre&nbsp;2026. Les exigences en mati\u00e8re d&rsquo;\u00e9quit\u00e9 et d&rsquo;engagement au niveau de l&rsquo;\u00c9tat sont d\u00e9j\u00e0 en vigueur.<\/p>\n\n\n\n<p>Les strat\u00e9gies qui feront la diff\u00e9rence en mati\u00e8re de satisfaction des membres, de fid\u00e9lisation et d&rsquo;efficacit\u00e9 op\u00e9rationnelle sont celles qui mettent en place une infrastructure capable d&rsquo;inciter concr\u00e8tement les membres \u00e0 agir, en s&rsquo;adressant \u00e0 eux dans leur propre langue et en les guidant vers des actions sp\u00e9cifiques ayant un impact r\u00e9el sur le plan financier et qualitatif, et ce, \u00e0 une \u00e9chelle que les centres d&rsquo;appels et les technologies traditionnelles ne pourraient jamais atteindre.
